Prevention services are designed to help families keep their children safely at home; the goal is to reduce the likelihood of placements into foster care or reduce a child's risk of re-entry into foster care after they return home.• Monitored a caseload of 14 families within the NYC ACS (Administration for Children's Services) Preventive Services system.• Assessed the safety of children living in at-risk homes by conducting home and officevisits• Provided individual and family counseling and support to clients with challenges related to mental health, domestic violence, substance abuse, etc.• Partnered with Child Protective Services, NYC Family Court(s), medical providers, and legal providers. • Referred clients to various entitlement programs including Medicaid, food stamps, andaffordable housing.
