Increasing capacity for a sustainable support network for the participants (parents/Carers) of children and teens on the autism spectrum. Also supported participants to link into existing community supports, services and how to access funding. SOUTHERN CROSS CARE INCAFTER HOURS COMMUNITY CARE COORDINATOR 2007 - 2011After hours coordination of community care client services, in the metro area and Broome. Supervise service provision during after hours, 430pm to 8am. Support and assist clients and staff with service delivery, scheduling, liaising with Health care services, Families, hospitals and pharmacies. Develop deliver and coordinate other therapy assistants in the delivery of meaningful activities within the facility and the community, including accessing and attending community based activities. Design and coordinate special events, special day programs. Coordinate and support culturally appropriate services to clients and residents from multi-linguistic backgrounds, including special day activities and traditions and foods. Deliver specialist intervention services to stroke patients, mental health patients, Alzheimer’s and dementia specific patients. Design weekly activity plans for both sites, carry out assessment, report writing and service delivery under the direction of the Registered Occupation Therapist and Site Clinical Psychologist.
