Revamped, created training for, and developed a premier relationship with Health Care Policy and Finance for the Colorado Choice Transitions pilot program– or Money Follows the Person program. This included being a member of the Colorado Advisory Committee for CCT. The program became permanent across Colorado for individuals being de-institutionalized or moving to less restrictive environments. By creating increased teamwork and understanding of job roles across the company, I aided in the Case Management integration with Physical Health Teams, Behavior Health Teams, and Long-Term Care to create a stronger and more cohesive case management department in preparation for the new RAE contract. Revamped and invigorated Resource Committee required by regulations and grew participation by ensuring remote access. Project Lead for Administration on Community Living No Wrong Door Grant to Colorado. Included coordination with the Aging and Disability Resource Center, Area Agency on Aging, Community Center Boards, Center for Independent Living, Department of Human Services and Single-Entry Point. Creating a shared database, 800# phone queue, training options counselors with person centered program, andcommunity public speaking and management of traveling teams and peer counselor. This work culminated with Colorado moving to redesign its case management agency structure. In collaboration with the Organization Learning team, we created companywide leadership training on "Resilience."
