This job used the collaborative care model, in which the client is supported by their PCP, the in-house psychiatrist, and the case manager. As a case manager, I worked with the client to meet the recommendation made by the in-house psychiatrist. As per recommendations, clients sometimes need to be referred out for more intensive services; I would assist with that. In other situations, the client needs coping skills and the recommendation was that they would work with me through therapy. At times, I had 60-80 clients, whom I saw monthly, biweekly, or weekly. I worked with clients with mood disorders and adjustment disorders. I conducted monthly GAD and PHQ assessments, assisted clients in setting goals, and supported them through education about their diagnosis. Resources that were introduced were self-compassion, Mindfulness, CBT, coping skills, meditation, breathing exercises using videos, worksheets and lesson plans.
