Program Coordinator - 18-21 Transition Program
Current- Develop and implement tailored curriculum and instructional strategies to meet the diverse needs of young adults transitioning from school to adult life- Facilitate vocational training, independent living skills development, and community integration activities to empower students for successful post-school outcomes- Collaborate with students, families, Child Study Team members, and external agencies to create individualized transition plans, ensuring alignment with students' goals and aspirations- Provide guidance and support in navigating post-secondary education options, employment opportunities, and accessing community resources- Conduct assessments to identify students' strengths, interests, and areas for growth, informing personalized transition planning and skill-building initiatives- Organize job shadowing, internships, and work experience placements to provide real-world exposure and practical skill development opportunities- Implement transition-related instruction on topics such as financial literacy, self-advocacy, and transportation planning to foster independence and self-sufficiency- Maintain accurate records and documentation of student progress, transition activities, and community partnerships to inform decision-making and program evaluation- Foster a supportive and inclusive learning environment that encouraged self-confidence, self-determination, and positive social interactions among students- Incorporate assistive technologies and adaptive materials into instructional practices to optimize student engagement and facilitate personalized learning experiences, ensuring inclusivity and accessibility for all learners
