I truly believe in the PACE model, all-inclusive health care under one roof with the goal of keeping vulnerable adults in their homes. I witnessed people begin to thrive once under the care of their PACE team. My time at PACE was spent as a social work supervisor. I supervised various teams, including the social work team, and MSW interns. I also managed grievances, appeals, and quality processes for the site. I trained staff and implemented a peer led support group to help staff that were struggling with the loss of Participants. I co-authored and implemented PACE’s Older Driver’s Policy with our functional manager. I also engaged Participants in group therapy and activities that promoted cultural diversity.

I spent many years visiting homebound, medically fragile individuals all over Southern California. I served patients of all ages (newborn to over 100). Some of the patients I served were on hospice. I provided counseling to patients to help them cope with their illnesses and functional limitations and to their families and caregivers to help them cope in their support roles. I provided education and linkage and intervened when there was crisis. I worked closely with the home health agencies that referred the patients and had the privilege of training new social workers on engagement, counseling, and case management techniques.

Newhall, Ca

I initially started with Single Mothers Outreach (SMO) as an intern. Once my internship was completed, I was hired as a case manager to continue doing the same work. I provided education and linkage to single parents living in Santa Clarita. I facilitated individual and group therapy. I frequently attended city council meetings where I advocated for the housing needs of single parents in the area. I also collaborated with the local college’s EOPS Department to support single parents who were enrolled in classes. During my time at SMO, I was asked to assume the role of case management supervisor while my supervisor was on maternity leave. In addition to my normal duties, I supervised MSW interns and volunteers, conducted program evaluation, and monitored grant funds.

Montrose, Ca

This has been the most challenging role I have been in as a social worker. This role also strengthened my commitment to social justice. Individuals who are admitted into long-term care are normally experiencing great difficulty, so the social worker at a long-term care facility plays a vital role in the betterment of these individuals. I spent my time at Verdugo promoting resident rights. I advocated tirelessly for quality care and dignified discharges in interdisciplinary meetings and in meetings with the administrator. I worked closely with the Ombudsman, Department of Public Health, law enforcement, and the Public Guardian to assure the safety of residents. I facilitated group therapy for a large group of residents on a weekly basis. I brought Alcoholics Anonymous to the facility for support of residents who were struggling with alcohol and substance use but could not leave the facility due to medical concerns. I conducted assessments of all residents, supervised social services designee, and monitored the department budget.
