Complex care coordination for people with multi morbidity and high readmission rates. Involved working with a broad range of stakeholders at all levels to ensure appropriate integrated management plans were put in place spanning secondary and primary care. A broad range of clinical knowledge across long term conditions was required, along with the ability to provide self-management support to patients and their families. The role also required an ability to coordinate specialist input and face to face meetings to avoid duplication of care, whilst maintaining clear roles and responsibilities across all providers. Education and support was also provided to acute and primary care staff. The ability to identify potential risk, deal with incidents and complaints in a timely and sensitive manner were crucial to this role.

Worked with remote clinics and communities to implement best practice chronic disease management and prevention.

Developed chronic disease management plans with patients which included goal setting and self-management support. Also provided primary health care nurse service 2 sessions a week and managed visiting specialist clinics.
