Personal Services was a program within the VNA of Manchester & Southern NH.I managed private duty in home services; clinical staffing and management for area assisted living community and provided coordination of care for patients/families needing long term care services.

Senior Care Management was a program within the VNA of Manchester and Southern, NH. I provided assessments and created care plans to assist patients to remain in their homes/environments for as long as possible. I provided support and education to family members and the community on senior care management. As a member of the National Association of Geriatric Care Managers, and interim Director the NH Chapter of NAGCM, I collaborated with community partners to ensure services were available for those in need in our communities.I managed a program that provided assistance to indigent women in the Manchester area through a grant funded by a local philanthropic group. I coordinated both medical and non medical services that the women needed to maintain their independence while remaining safe in their homes.

I provided care management to elderly or adult-disable individuals who qualified for a waiver program funded by the State of NH Medicaid system. I worked closely with many community agencies to ensure service plans were meeting patient needs.

I worked as a Staff Nurse at the Elliot Hospital from 1988 to 1991. In 1991, I began working for Elliot Home Care (has had some name changes and is now is VNA of Manchester & Southern NH) as the Office Intake Coordinator. Over the years, until 2003, I had a number of roles within the VNA to include Clinical Manager- home care, Utilization and Review Nurse and Agency Relations Manager. In all of my roles I assisted the agency in working with community groups as well as outside physician practices to provide care to those who were homebound. I was a member of many community groups to include the Manchester Chamber of Commerce, the Area Agency on Aging Committee as well as some local community groups that worked to provide a safety net for those most vulnerable in our community.
