Summary of Qualifications• Clinical practice diversity: Acute Care; Critical Care; Rehabilitation Care, Stroke Care and Geriatric Care – knowledgeable in navigating system to flow patients to most appropriate level of care • Leadership/Management experience focused in Rehabilitation Care, Complex Care and Therapy Services delivery to support inpatient and outpatient programs• Change management experience focused on implementation of best practice initiatives and service delivery redesign• Regional Rehabilitation Network (RRN) member engagement in the assessment and planning of regional rehabilitative services to support centralized, standardized referral processes and improved access to the “right level of care, at the right time”• Partnership development and establishment of collaborative working relationships with; Community Care Access Center, Adult Recreation Therapy Center, Grand River Community Health Center, Landsdowne Children’s Center, local Retirement Homes, Long Term Care facilities and Family Physicians to support seamless transition of care• Systems perspective and “Big picture” view of healthcare service delivery, chronic disease management and interdependencies across the continuum of care
