I was hired as a registered nurse to assist the organization in achieving Patient Centered Medical Home recognition through development of programs designed to help patients achieve their health goals.I worked with healthcare providers, operational leaders and support staff to develop a comprehensive diabetes education and case management program utilizing clinical best practice guidelines. I utilized i2i Tracks and analyzed data to determine which patients would benefit from diabetes education, support, and case management.Utilizing motivational interviewing, I worked with patients and their families to develop achievable plans to assist patients in achieving their healthcare goals. With my support and encouragement patients were able to bring their Hemoglobin A1c's into ranges indicative of good glycemic control.As the Nurse Lead I spent time, with my team and patients, to ensure our processes and workflows met the needs of the organization and our patients.

When I ran my own business I took pride in capturing the beauty in each and everyone of my clients and it was my goal to make the photo experience the best they ever had. I utilized online sources to learn photoshop so I could enhance the natural beauty of my clients.I developed a website and marketing materials, for my business.I loved consulting with my clients, re: coordinated outfits and photo settings.
