Leena Mehta Email and Phone Number
I was raised in India by a family of doctors and was deeply immersed in the healthcare environment from an early age. After migrating to the United States, I quickly realized that I could contribute more effectively in Healthcare Management. Consequently, I pursued an MPH and an MBA in Healthcare. As a trained physician from India, I have applied my medical knowledge and expertise to develop comprehensive Disease Management programs for the 'Top Five' chronic conditions. My strategic approach in building relationships and leveraging my strengths has created opportunities to propel proactive management of high-cost chronic conditions. As an experienced health management leader, I have successfully driven multi-million-dollar performance-based incentive programs to completion within our fragmented healthcare industry. I have led initiatives for Patient-Centered Medical Homes and guided cross-functional teams to enhance HEDIS quality outcomes. My approach emphasizes innovative Population Health Management and referral services to support providers participating in Value-Based Care. The transformation to value-based care will take time. However, I believe that primary healthcare is a basic necessity, akin to housing, food, and clothing. It is essential that healthcare be affordable and meet minimum quality standards, ensuring that when any of us need it, we can access care that enables us to lead full, productive lives.
