With The Salvation Army New Hope Leslieville: • Admitted and discharged clients• Facilitated intake and triage processes for clients by completing an intake interview in a private space, collecting required information to determine level of service needed• Provided appropriate information and resources, oriented residents to hostel regulations and schedules as well as all available services during the intake process• Ensured resident admission and discharge information is recorded in appropriate places• Assisted with supervision of clients• Provided practical and emotional support, assistance, and encouragement to clients• Performed all necessary clerical duties for documentation, including SMIS, Hostel Registry, triage, logbook entries, and all other required forms• Made necessary internal referrals to case management department or pastoral care• Made necessary/appropriate external referrals to community resources to assist clients in meeting their basic needs and assisted in moving towards housing and housing stability• Supported clients in securing identification and completing taxes and gaining income as identified on the intake triage• Monitored the length of stay of clients and notified case management team of significant stays and/or changes in client behaviours that may require greater support• Supported Level 1 clients in navigating basic social service supports • Sorted and processed resident mail on a daily basis, stamped and distributed all mail, redirected all mail for residents who were no longer at shelter• Reported all building problems to the Assistant Director or designate• Ensured all donations received after business hours were received and processed according to policy• Ensured that all telephone calls were dealt with professionally and appropriately • Supported clients in following agency rules and expectations consistently
