Initiates referrals as needed toother health team members when delegated by the Case Manager● Skills in wound care including negative pressure devices, tracheostomy care, colostomyand IV infusion, care for PICC lines and discontinuation of lines.● Reports significant changes in client status to physician and other members of the teamin a timely manner consistent with client needs● Implement skilled interventions with a focus for achieving realistic client outcomeswithin a specified time period manages patient medication● Collaborate with physicians, other health care professionals (therapists, social services,pastoral care, supportive services), clients, and families in developing a comprehensive,coordinated plan for care.● Supervises applicable home care team members as delegated by RN Case Manager
