Social Prescriber
Social prescribing involves helping people to improve their health, wellbeing and social welfare by connecting them to local community services and activities.I accepted referrals from GPs, Health Practitioners and self referrals. I worked with people to explore options to solve or improve their non-clinical problems. Social prescribing is based on the belief that the best way to tackle around 70-80% of health issues lies in non-medical solutions. The main purpose of my role was to provide individual support for patients to access a range of health, social care, welfare advice and other support services by setting goals and priorities with each patient to improve their health and wellbeing.I carried out initial assessments and jointly identified goals and priorities with each person. I referred and signposted these clients on to other services which were appropriate to meet their needs. This may have been health and wellbeing services or other services that deal with the wider determinants of health (benefits, housing, education etc.) and provided ongoing support and guidance to clients (utilising motivational interviewing techniques) during the sessions.I supported people to build a comprehensive network and knowledge of wider support services for people with high support needs including: social isolation/loneliness, wellbeing, housing, unemployment, welfare benefits and contribute this knowledge to a local directory.I engaged across the identified GP surgeries on a regular basis.I worked as part of the team within the surgeries, attending team meetings and liaised with healthcare professionals to increase visibility of the service and ensured information was up to date and relevant.I worked as part of the Adur and Worthing Wellbeing Team, attending team meetings, raising awareness of the project and ensuring referrals and feedback mechanisms to the team were effective.
Floating Support Officer - West Sussex Homelessness Prevention Partnership
I was responsible for and managed my own caseload of clients who were at risk of homelessness. These were families or single people. I accepted referrals from the Local Authority Housing Department who identified a need for support to attempt to prevent loss of tenancies. These could be PRS or RSL accommodation. Families are often affected by the Benefit Cap, leading to significant rent arrears and a threat of eviction. I supported by first assessing their needs then liaised with landlords, benefit agencies and other relevant external agencies, where necessary, as well as providing information about budgeting and welfare benefits. I created action plans with the client to enable them to develop long lasting skills that will empower them to deal with future situations. These clients could be dealing with challenging issues and were often experiencing emotional distress. I was often required to deal with several challenges at the same time and was able to maintain a professional, calm and positive approach. I met with clients in their homes, the community or an office environment. That required me to conduct and regularly review thorough risk assessments. I have the ability to respond to issues such as safeguarding adults at risk and child protection. I was required to maintain comprehensive records of all my support activity.
Floating Support Officer
I worked as a Floating Support Officer in West Sussex. I provided crisis intervention and preventative short-term support for people with pressing housing related needs. The assistance I provided enabled clients to resolve their current difficulties and develop their ability to manage living independently. From the referral to assessment and throughout the duration of support I worked collaboratively with clients using a person-centred approach. I supported clients to create personalised support plans with SMART outcomes that are meaningful to them. I supported clients holistically, not only to address their housing needs, but other issues such as income, health and employment, which together have a positive effect on their ability to manage living in their own home. Through a coaching approach and practical support I encouraged clients to develop lasting skills, which empowered them to deal with any future situations with confidence. I ensured that clients had the best opportunities to achieve their support outcomes and both refer onto and liaise closely with other specialist organisations
Support Worker - Stonham Directions, Justice Services
I worked as a Support Worker based in West Sussex. This was a referral and assessment service for ex-offenders. I provided support with housing, benefits, education and resettlement into the community. The service worked across West Sussex offering short term accommodation in shared properties.I reported to the Client Service Manager and was part of a team, responsible for the provision of housing and tenancy support to promote independent living to people wishing to resettle into the community. I have experience of providing a flexible and responsive service to people and effectively managed their support needs and any risks identified, with a proactive approach. Working within my team, I liaised with other agencies including the Offender Management Team, Probation, local councils and other, relevant, voluntary agencies as well as carrying out assessments at various HMPs.I am experienced working within a support/care environment and dealt with vulnerable people who have served a custodial sentence whilst working in a multi-agency approach.I have a good knowledge of the issues facing people with a background of offending behaviour, and am able to use my knowledge of resources, available in the community, to enable people to receive opportunities to engage and reduce the risk of re-offending.
Waitress
I have been employed as a Waitress/Kitchen Assistant. I was responsible for the smooth running of the kitchen while I was on duty. I had to ensure that products are produced to a high standard, timings were imperative, and hygiene and cleanliness were kept to a high standard. Whilst working in the restaurant, I had to ensure that all the customers were happy with a high emphasis on customer service. I have an excellent telephone manner and ensured that the restaurant was kept clean and tidy. I had to work well in a team and under pressure. I was ultimately responsible for monies paid to me.
Pre-School Practitioner
During my time here I have achieved a Certificate in Pre-School Education as well as having attended several training courses. I assisted in the planning and preparation of play opportunities. I needed to talk to parents and carers to ensure the child’s needs were recognized and met. I have found that I particularly enjoy the challenge of working with additional needs children and find this very rewarding.
Support Manager
As a duty manager I was responsible for the smooth running of the restaurant during my shift. This involved managing staff and any issues that arose, i.e. covering absences. I was also responsible for customer satisfaction and resolving any problems in a professional and friendly manner. I checked and ordered stock. Tallied daily takings and banking. I had to ensure cleanliness and hygiene levels and dealt with any issues arising from faulty equipment. I was also a key holder and therefore responsible for the security of the premises.
