Clinical Quality Manager
CurrentResponsible for the development, implementation, and maintenance of a primary care performance improvement program for 30+ practices, including ongoing monitoring of outcome measures, clinical standardization, and outcomes improvement initiatives. Coordinate clinical data collection, with special emphasis on leveraging electronic health record applications. Act as a change agent to promote the development and integration of systems thinking and the use of improvement tools and methods in interdisciplinary and departmental teams. Collaborate with medical staff, clinical staff, and operations to achieve improvement in all domains of patient care. Manage payor-sponsored payment-for-value programs, local health reporting initiatives, and regulatory/government programs, such as Meaningful Use, PQRS, and D5 public reporting. Support NCQA Patient Centered Medical Home (PCMH) recognition program. Serve as quality lead for sixteen practices participating in the Comprehensive Primary Care initiative (CPCi), a CMS Centers for Medicare and Medicaid Innovation (CMMI) project. Provide patient safety leadership for primary care and specialty offices.
