CurrentMedication reconciliation of recent admissions and patients that present in the ED.Effectively communicate with other healthcare providers and complete med rec in a timely manner for pharmacy orders to be approved as well as to allow for problem solving and effective patient care. Assess medication accuracy and clarification of dosing under pharmacist supervision. Assess medication appropriateness and suggest course of action for escalation or de-escalation of therapy under pharmacist supervision. Supports daily pharmacist activities including PRN dosing, SUP medication verification, and Lovenox dosing reporting. Support daily pharmacy activities including filling Pyxis and managing inventory. Distributing controlled medications and managing inventory of controlled medications. Pre-packaging of medications for Pyxis refills and stat orders. Non-sterile, sterile, and hazardous compounding in accordance to USP 795, 797, and 800 respectively.Help streamline outdated medication processes within the pharmacy to allow for reduced time spent on removal of expired medications and to reduce medication waste.
