My primary responsibility is to ensure the delivery of exceptional healthcare to every resident under our care. This involves conducting thorough nursing assessments, actively participating in the development and execution of individualised nursing care plans, and proficiently overseeing medication administration. I also serve as a crucial point of contact, fostering effective communication between the facility, medical practitioners, and family members. Throughout my role, I consistently strive to provide nursing services that empower residents to maintain their independence, preserve their dignity, and enhance their overall lifestyle.

Virtual Covid Ward - providing support via telephone and telehealth to patients that are positive for Covid-19 while they are in home isolation. Conducting admissions, daily patient reviews, discharges and escalating to Medical Officer when required. Providing guidance on what to do and expect during the isolation period and advice on what to do in case of any concerns. Rotational on-call required.

Leading case management of Home Care Packages Level 1 to 4, upholding clinical governance and managing the quality management framework within Home Care services to ensure care standards are met, coordinating the planning and implementation of a Model of Care, providing direct and indirect clinical care to customers within the scope of practice, ensuring customer records, incidents and complaints are documented and maintained correctly and participate in any investigations, also providing guidance, support, and direction to staff in nursing care and service delivery on shift as required.

Being part of the School Immunisation Program for the High Schools based on the Sunshine Coast as a provider for the Sunshine Coast Public Health Unit as well as involved in the Influenza Immunisation Clinics leading the team on several occasions. I manage a team of registered nurses when covering the Clinical Nurse role. My responsibilities include assessment and reviews of Home Care Packages, Assessment and case management of CHSP, DVA clients, Transition Care Program and Post-Acute funded clients. Performing admissions and discharges of clients. Providing complex wound care management, such as VAC & PICO dressings, processing of MASS & CAPS applications, assisting clients with their medication requirements, insertion, and management of catheters, management of Rocket drain, stoma care, monitoring of palliative care symptoms, development of personalised care plans, providing education to clients and family members, consulting with internal and external services to provide clinical solutions, and also providing training of new registered nurses.

Registered Nurse in charge of organisation and implementation of nursing cares provided to residents by nursing staff on shift. The facility is a 55 bed home offering high care, dementia care, respite care and palliative care.

Provided hospital care in the home by delivering a range of clinical service including complex wound care, IV Antibiotic Therapy, administering other medications, PICC Care/Dressing/Removal and Catheter care. Liaising with the medical team from the hospital. Identifying & contributing to the development of patient educational needs. Maintaining patient records updated.

Data collection and data entry (including completing geriatric assessments) for a research study entitled “Telehealth for Residential Aged Care Facilities: A Pragmatic Randomised Controlled Trial”, funded by the National Health and Medical Research Council (NHMRC) and was conducted at 10 Residential Aged Care Facilities in Brisbane Metropolitan Area, Logan City and Moreton Bay Region. Liaising with members of the research team and the residential facility staff for the geriatric assessment for this research project. Maintain accurate and comprehensive data collection records. Interviewing patients and families. Review of medical records and documentation.

Assessment and case management of HACC clients, including specialist duties such as wound management, continence assessment, medication administration and management and any other duties as required under the relevant funded programs guidelines. Providing periodic supervision of clients' medication and medical status where lack of knowledge results from mental, linguistic or cultural problems. Providing input in the education and training of Community Care Workers, Volunteers and Informal carers on basic skills in the areas of interpersonal relations, recognition of problems, use of lifting equipment, assistive devices and the personal protective equipment (PPE) and general health issues.

Providing direct and individualised nursing care to 72 residents on a daily basis in a defined clinical area. Adhering to holistic and client-centred care by promoting the client’s autonomy with daily living activities and making decisions about the care. Effectively communicating to line manager and other relevant staff any information/issue/concerns relating to residents, relatives, visitors and staff. Providing dignified, respectful and culturally safe care to families during critical times such as palliative care and the passage from life to death.
