• Delivers direct patient care; assesses patients physical, psychosocial and/or emotional needs; provides patient and family teaching; responds to phone messages from patients and informs physicians as needed• Administers medications as assigned, serves as a resource person for knowledge of medication uses, side effects, and dosage, checks for proper medication order and documents medications appropriately, obtains required consent forms to perform physician ordered procedures. • Updates physician on patient status and changes in the acute care setting, advocates for patient rights and educates patients regarding new diagnoses.• Performs neurological checks and stroke scale assessments using the NIHSS scale as ordered by physicians.• Assist in providing hygiene and basic care to patients unable to care for themselves.• Remain vigilant to patient status changes to update physicians and change treatments as needed.• Training as a member of the skin resource team to help manage and prevent pressure ulcers, and to promote wound healing.• Vice chair for our Unit Based Council which focuses on creating guidelines for new policies being implemented within the hospital, supporting shared governance, implementing programs to improve HCAP and Press Ganey scores, promoting unit safety, implementg evidenced based practice, creating goals for the unit to support team work.• Attend courses focused on computer training to support floor staff when computer issues arise.

• Assess client’s health care needs and coordinate with community medical resources to obtain needed services.• Serve as a member of the Interdisciplinary Team (I-D Team), Human Rights Committee, and review all applicants for admission. ( Annual & Semi-Annual ISP Nursing Review, 60 day Dr.’s Rounds, accompany client’s to doctor’s visits as needed to discuss plan of care and attendance to Management Meetings• Develop a Health Care Plan (Nursing Care Plan) under the guidance of the attending Physician for each client. Review and Update the Nursing Care Plan Quarterly• Provide written progress notes regarding health care objectives, and on-going charting of changing medical needs.• Conduct training to direct care staff in the areas of administering medications• Certify the proficiency of staff to administer drugs, and carry out treatments following an approved course in “Medication Training.” Monitor staff performance and issue counseling notices as needed. Provide an annual evaluation of staff performance in the areas covered by the “Medication Training” course.• Provide review of medication documentation for each client every two (2) weeks with record kept of the reviews. Assure that proper procedures for keeping medications locked, proper disposal of medications and record keeping are being maintained.• Conduct in-service and training with direct care staff on signs and symptoms of illness, infection control, skin care, menu planning, and management of emergency medical procedures, seizure control, bowel and bladder control• Be available to staff to review clients on a daily basis if necessary.• Order adequate amount of medical supplies and stock medications.• Work with direct care staff in the areas of positioning, skin integrity, assisted feeding techniques, and dietary needs.
