My responsibilities include bridging services between our Nurse Practitioners and the Nursing Homes and Assisted Living facilities that we serve. Our NPs provide exceptional, geriatric specialty care focused on medication management with people who have these diagnosis, residing in these facilities. Additionally, I provide specialized training for CNAs and PAs to assist them in better managing challenging behaviors of people with psychiatric and dementias diagnosis.

These cooperatives in turn become part of the business negotiations and are empowered to make decisions about their direction and future.

I provide Health Education and referral to ongoing services where appropriate, to employees of companies of all sizes. The goals of these interventions are to reduce participant absences, improve individual morale and increase participant productivity.

I provided family support and education to family members caring for people afflicted with all forms of dementia, including but not limited to Alzheimer's disease. I designed and implemented dementia specific activities and assistance with socializing to small groups of people with dementia.

My position duties included case management with 15 severely mentally ill residents including individual, family and group counseling. Additionally, provided milieu counseling with all 54 residents of the two locked, long term psychiatric units at the facility. All computer documentation connected to these counseling duties was also my responsibility.
