As a Care of the Elderly family physician and Schlegel Research Chair in Primary Care for Elders, I am passionate about improving healthcare for older adults living with dementia, frailty and other conditions associated with aging. I have spent the past 30 years practicing family medicine and caring for older adults living with dementia. Early in my career, I recognized gaps at the primary care level in how people with memory problems receive care. Together with my team at the Centre for Family Medicine, to address these gaps I created a unique training program and care delivery model called MINT Memory Clinics (previously known as Primary Care Collaborative Memory Clinics). With positive feedback from patients, caregivers and healthcare professionals, there are now more than 100 MINT Memory clinics operating in Ontario and our model continues to spread. I am also active in the research community and have collaborated with colleagues on numerous studies and publications on dementia, frailty and other conditions related to aging.
