• Complete in-home/on-site Home Safety Assessments, Mobility Assessments, Cognitive Job Coaching, Personal Care Needs Assessments, Progressive Goal Attainment Program (PGAP) Assessments, and Ergonomic Assessments.

Lead a team of Occupational Therapists, Physiotherapists, Social Workers, Dietitians, and Rehab Assistants who are providing care to clients in a community setting.

• Offering private Occupational Therapy services in the Ottawa area. Primarily focusing on ergonomic assessments, home safety assessments, mobility aids assessments, and providing the Progressive Goal Attainment Program (PGAP).

• Assist in managing a team of over 70 staff• Manage a dynamic and busy sales floor• Monitor sales metrics and inform team regarding goals/progress towards stated goals• Establish supportive and productive relationships with all team members• Collaborate with team members to ensure optimal guest experience and support store operations

• Complete functional assessments in an acute care setting, primarily on the stroke unit• Establish client centered goals and make recommendations for ongoing medical or rehabilitative care • Complete referrals for community services to facilitate safe and appropriate discharges

• Provide thorough assessment of the client’s functional ability at home and in the community• Assess the client’s equipment needs and implement the necessary equipment at home• Develop treatment plans to help individuals reintegrate into meaningful activities • Complete detailed report writing on progression of client’s functional status

• Provide comprehensive multidimensional screening assessments for older adults with complex needs in the community • Identify the older adults’ biomedical, psychosocial, functional, and environmental concerns• Communicate findings to the older adult, their support network, and health practitioners• Facilitate recommendations to optimize the older adults’ health and functional independence in the community
