This position will be 1 of the 2 supervisors over teams of Disease Intervention Specialists (DIS). The DIS teams follow up with patients both in clinic settings and in the field (e.g. homes, employers, physicians’ offices, other community agencies). The DIS Public Health Follow Up program is required to meet specific requirements and strict deadlines.Essential duties and functions, pursuant to the Americans with Disabilities Act, may include the following. Other related duties may be assigned.Conduct planning, development and evaluation of assigned program.Provide supervision of program implementation.Perform grant writing and management of grant budget.Collaborate with community based organizations.Promote program and provide program information to community.Provide educational and community awareness presentations.Knowledge of the area of specialization.Knowledge of current principles and practices of planning and program implementation.Knowledge of organization and administration of public health and/or human service programs.Knowledge of program monitoring and policy analysis.Skill in effective oral and/or written communication.Skill in resolving problems or situations requiring the exercise of good judgement.Skill in establishing and maintaining good working relationships with other City employees and the public. -
