• Perform nutrition assessment for critically ill patients in a 42 bed ICU and CCU using the Nutrition Care Process in ADIME format.• Develop and implement an individualized plan for nutrition intervention in accordance with the patient’s nutrition prescription which usually requires development of a therapeutic plan for specialized nutrition support including enteral and parenteral nutrition.• Evaluate the effectiveness and measures progress towards outcomes of medical nutrition therapy interventions. Reassess and implement changes as needed.• Educate patients and their families on nutritional needs and/or diets; assess patients understanding of and compliance with prescribed diet principles.• Provide personal or phone contact and follow-up with patients and physicians as needed.• Assist in achieving compliance with The Joint Commission standards and other accrediting organizations and with regulatory agency standards, which may include state and federal.• Act as an advocate, resource and liaison with Nursing and Medical staff to provide optimal communication and enhance patient nutritional care.• Assist in the education of dietetic students and interns. • Participate in departmental and interdisciplinary meetings.• Keep up-to-date on developments in critical care nutrition and currently planning to test for Certified Nutrition Support Clinician in May of 2015.• Previous experience working on an interdisciplinary bariatric team providing education for patients undergoing bariatric surgery.• Previous experience in NICU and Pediatrics.
