The position required me to work over a one year period with clients who were between the ages of 18 and 24 and were dually diagnosed with an Axis I mental disorder and an Axis II diagnosis of mild mental retardation. I was assistant to the main supervisor and performed assessments to determine eligibility for the program, and upon acceptance developed and helped to implement treatment goals in multiple domains including medication management, social skills training, and preparation for the transition to independent living. I prepared quarterly service plan reviews to assess readiness and at the time clients were determined to be ready to move to another level of housing I submit housing applications to the NYC Human Resources Administration. I also performed duties such as advocacy in the legal system, assisted clients with obtaining and maintaining benefits, and performed outreach services with discharged clients.

As an intern at a 53-bed psychiatric unit as part of obtaining my Masters in Social Work, I was responsible for assisting one of the three LCSWs assigned to individual cases by doing psychosocial assessments, facilitating family meetings, writing weekly case notes and measuring objectives, and doing discharge planning to prevent relapse and recidivism and assist patients in achieving stability in the community by providing them and their families with resources. I also participated in attending weekly grand rounds with guest speakers on topics such as case studies, psychotropic medications, forensic psychiatry, and properly using the DSM-IV-TR for diagnosis.

At this position I was responsible for planning and implementing treatment goals for 4 of the 12 consumers in a residential setting. I had weekly meetings with clients to assess and measure progress and wrote case notes and service plans to determine current functioning. I assisted clients with facilitating communication with case managers and with preparing for moving to a less restrictive level of housing by developing coping skills, medication management, ADL skills and treatment compliance.
