I started at the SET Clinic as the Ex-Offender Case Manager where I coordinated care for patients with a Felony history. I visited offenders in the county jail and half-way houses to educate them about SET and the services available through the Ex-Offender Program (CHRP) and assist with preparing them for their release back to society. I developed curriculum for Employment Readiness (resume building and interviewing) and Budgeting as these classes were going to part of the planned second phase of the program. My duties expanded as I was asked to develop and facilitate SET's new staff orientation. I was the direct liaison between the medical clinic and the maintenance department and cleaning company, participated in the weekly Lean Processing meetings, assisted with the education and compliance in regards to HIPAA, and wrote the monthly volunteer and patient centered news articles. Currently, I provide three fronts of case management to all of SET's patients who are in need of services beyond their basic clinical needs. I assist with the medical, social and mental health needs of our patients. I work very closely with the medical providers to coordinate the needed patient services, provide community resource information, assist with referrals, medication refills. I coordinate the patient education programs, help with grants when needed, enter clinic data for grant writing purposes, assist patients with the Medicaid application process (CAAS) and coordinate all Medicaid prior authorization requests. I routinely assist the front and back office (scheduling, check in/out, money collection, charting, etc.) and run the homeless clinic when needed.Additionally, I work with the HEALS Mental Health Court in the capacity of assisting with coordinating care and applying for Medicaid. I am a member of the Peak Military Care Network Committee, the Colorado Emergency Preparedness Partnership, and the Community Partnership for Child Development Health Services Committee.
