o UC is Responsible for for identifying and building a network of Private Qualified health care providers/Private Hospitals/Private Labso And Coordinate their engagement towards identification. Notification and treatment of all forms of tuberculosis patients especially in KAP (key affected population)o Mapping of Key affected Populations in urban areas and Coordinating active case finding (AX. Samvad) amongst themo Identify and Map Specialists-MBBS Doctors, AYUSH, Private Labs and Private Hospitals in assigned urban areaso Sensitize and Advocate with private providers for complying with STCI guidelines and notification of all form of TB patientso Ensure engagement of identified private health care providers in Axshya activities.o Coordinate with RNTCP for implementing project Axshya activities in urban settingso Ensure Monthly Collection of data/reporting forms (Notification) from all engaged private health care providerso Supervise Community Volunteers (CVs) to ensure regular follow-up with the TB Patients identified through private health care providers for treatment completion.o Responsible for timely submission of reports for the project activities.
