Clinical Case Managers provide case management services for severely emotionally disturbed children of differing ethnic backgrounds who are placed in treatment foster homes. Caseloads for Clinical Case Managers average 12-14 youth and approximately 5-7 treatment foster homes. Case management services include a weekly face-to-face contact with the foster parents as well as with each foster youth on the Clinical Case Manager’s caseload. In addition, Clinical Case Managers are required to coordinate educational, mental health, community and any other services pertinent to the youth’s treatment plan. Clinical Case Managers are also required to develop and implement a treatment plan for every youth on their caseload.In addition to my duties of Clinical Case Manager I also provide therapuetic services to a select number of clients. The focus of therapy is Trauma Focused Coginitive Behavior Therapy.

Therapist for youth placed in Intensive Services Program homes.The types of therapy utilized include Trauma Treatment, Dialectical Behavior Therapy, individual and group as well as family therapt.

Worked with youth in out of home foster care and treatment foster care placements as well as helping them reunify with thier families once out of home. Court services, case monitoring, case management, crisis services. Also conducted court ordered custody studies for family court.

Worked with Chronically Menatlly Ill adults living in community settings to ensure they could meet their social, emotional, physical and mental health needs. Developed treatment plans, provided psychotherapy, coordinated services, and general case management services.
