Rn Case Manager And Nurse Educator
Current• Manage a caseload of 55-60 clients; meeting with clients one-on-one in their homes. • Perform admissions, resumption of care, re-certifications, and supervisions of Home Health Aids, PCA, and Homemaking Services. • Create and update each client’s plan of care. • Educate clients on their specific needs to reach goals and outcomes. • Responsible for wound care, catheter changes, injections, venous blood draws, wound vac therapy, pic line dressing changes/blood draws/antibiotic administration, and INR checks. • Train new nurses over a 12-week span as a Nurse Educator. Accomplishment: • Successfully meet weekly quota, generally surpass it by 1-3 visits.
