Responsible for facilitating the rapid re-housng of homeless individuals and families who have been approved by the City of Chicago Low-Income Trust Fund or families residing in other supportive housing. Provide support to housed clients as they make the transition to independent living; assist clients in establishing linkage services in their neighborhood with the goal of preventing the return of the issues that led to their homelessness.

Marketing to nursing homes, HR generalist, recruit, coordinate/schedule groups and individual thereapy session with doctors, billing and program audit, facilitate groups and manage transportation portion of the company.
