Current• I work directly with the clinical coordinator and hospital case managers to admit new dialysis patients • Review hospital paperwork, prescription requests, and transportation forms and forward it to the appropriate teams• Set up transfers and travel for patients• Meet with interdisciplinary team to discuss patient needs and identify focus patients• Directly involved in patient care with Spanish translation• Educate new patients through SmartStart, using Clinical Management Tool to track patient progress in modality choice pathways• Update missing treatments and log hospitalizations correctly to improve clinical scores• Create weekly patient schedules and reschedule missed treatments • Print patient consent forms, including admission paperwork, vaccine forms, etc.• Help patients set up transportation• Enter new patient data and update insurance information• Create and update patient charts• Work with CMS Crownweb to submit ESRD 2728, 2746, and other clinical data• Assist with daily laboratory duties: printing and preparing labs, using centrifuge, packing and mailing labs• Coordinate with kidney transplant programs to ensure that HLA labs are drawn monthly• Order office supplies, organize teammate lunches, improve teammate morale, and help with other administrative needs• Work with interdisciplinary team to create engaging educational events and holiday celebrations with the patients• Put in work orders and service requests• Answer the phone in a fast, friendly, and professional manner• Reroute calls to appropriate department• Greet all patients in a warm manner• Screen visitors for Covid-19 symptoms• Help PCTs bring in patients and lead them to their chairs• Clean and decorate the lobby• Worked the cohort clinic for 9+ months, treating as many as 30+ Covid positive patients at any given time during the height of infection
