Working for the ambulance service, in a role similar to that of front line paramedic, within the scope of practice of a registered nurse.

In this role I have predominantly worked in the A&E setting however since completing my secondment in ITU, I am now able to also do shifts in this setting also.

I worked in all areas of the department, those being Majors, Resus, Integrated Front Door (IFD) and the Clinical Decisions Unit (CDU). Triaging patients, taking initial observations, ECG and bloods, escalating as appropriate. Carrying out further diagnostic and therapeutic interventions are requested by the assessing doctor. Escalating length of stay and factors affecting the length of stay to the nurse in charge to ensure breaches are avoided and minimised. I was in the process of getting signed off on the IFD clinical skills for example, suturing, plastering and requesting x-rays, with the majority of these skills being signed off.During the Covid 19 Pandemic the running of the department and management of patients and the appropriate clinical areas for them changed drastically, I have also worked in all of the clinical areas during the pandemic.I also have 2 link roles within the department, those roles are Green Champion and Major Incident.

During this secondment, I have looked after HDU and ITU patients. As I build up knowledge and skills in this specialty, when appropriate, I have been supported by other nurses when the patient's needs exceed my scope of practice and competency. This was during the Covid 19 pandemic.

In this role, while on shift I could be allocated to run Resus, Majors or the Integrated Front Door (IFD). When doing this, as well as giving patient care, I also have an oversight of the patients in that area to ensure patients have a plan, and they are referred efficiently, reducing breaches. As I developed in this role I took on the task of being nurse in charge of the shift. Whilst on secondment to this role, there has been the added challenge of the Covid 19 pandemic. During the Covid 19 Pandemic running of the department and management of patients and the appropriate clinical areas for them changed drastically, an example being that the Clinical Decisions Unit was changed into Majors and then later changed in to the Respiratory Assessment Zone. I have worked in all of the clinical areas during the pandemic. As well as giving patient care as described in my band 5 registered nurse role in the A&E Department, in my band 6 Junior Sister role, I also have an oversight of the patients in that area to ensure patients have a plan, and they are referred efficiently, reducing breaches. Ensuring the area is safe for both my colleagues and the patients.

Working in the inpatient unit, providing palliative holistic care to those with life limiting illness. Allowing my patients to live as well as they can, for as long as they can. A large part of my role was symptom control and emotional support as well as end of life care.

I carry out, coordinate and lead the provision of the highest standard of person centered care, through the use of good evidence based practice; working independently and as part of a team. In providing care I ensure the safety of the resident, my colleagues and myself whilst maintaining efficiency dignity and confidentiality. I have the ability to respond to sensitive, potentially volatile situations and distressed residents, family/friends and staff. I work as part of a team and independently, to assist individuals with dementia, to carry out their ADL's (for example feeding, personal hygiene, dressing & socialising), and where appropriate, promoting their independence. Providing a safe environment for them to live in, whilst respecting their dignity, confidentiality and remembering that I am in their home.

I have worked in the role from qualifying and have worked in both majors and minors as well as assisting with resus patients. I have successfully completed IV Skills for Beginners training and prior to obtaining my PIN, whilst in my Band 4 role, I completed Venepuncture and Cannulation training.

I do this by carrying out the daily care of patients whilst maintaining their dignity and meeting their specific requirements for example pre and post operativley. Activities I assist patients with include, personal care, nutrition and mobility. Changing dressings using aseptic techniques. As well as observations at regular intervals. Reporting my findings to the nurse in charge, and documenting all care given. I am also trained to do 12 lead ECG's. Shifts have included ITU and radiology.

During my time in this role I worked bank on Sheppard ward, which became Hestercome, in the same capacity as my permanent Health Care Assistant role in the same clinical areas. Whilst I was a student nurse I predominantly worked in the role of Health Care Assistant alongside the X-ray portering team.
