Development and oversight of the Discharge Planning Team for Kansas Health Solutions. Supervised Discharge Planning team. Developed processes and procedures and data reporting outcomes for the discharge planning team. Provided care coordination and discharge planning for challenging and complex members at state hospitals and psychiatric residential treatment facility for youth. Interfaced with Kansas Health Solution Providers and other community service providers to develop comprehensive post discharge service plans. Team Leader for Community Support Services Team that focused on Consumers who were homeless or struggled to maintain housing. Developed Transitional Housing Program funded by Federal Supported Housing Funds. Completed the Federal Grant applications to fund housing programs, oversaw the Federal Grants and completed quarterly and end of year reporting. Supervised the PATH Homeless Outreach Program- case managers serving chronically homeless mentally ill consumers. Completed the PATH grant and grant reporting requirements. Supervised all Bert Nash housing programs and staff related to housing programs. Competed all Federal grants, reports and licensing requirements related to each program. Performed Crisis hospital screens, clinical crisis interventions as well as clinical case management, therapy and DBT groups. Participated in : Governor's Subcommittee on Housing and Homelessness, Statewide Housing Specialist and PATH, City Of Lawrence Continuum of Care Committee.
