- Providers of Clinical Pharmacy Services to Primary Care, Secondary Care and Community Pharmacy- Independent Pharmacist Prescribers- GP practice support - Practice Pharmacist – daily management of all prescription related duties such as discharge summaries, daily prescription queries, day to day signing of prescriptions and medication reviews- Provision of anticoagulation services under Local Enhanced Service for GP practices- Clinical review services for chronic disease such as AF, Diabetes, COPD, OAB to optimise treatment regimens through face to face consultations with patients- Education to all HCPs and Pharmaceutical Industry (local and national)- Service development for community pharmacy- Enhanced DBS check- Presenting at local and national meetings and conferences- Management of a team of pharmacists and meds facilitator who work across numerous practices in differing roles

The United Kingdom Clinical Pharmacists Association (UKCPA) is a not-for-profit member organisation for pharmacists and health care professionals. It promotes expert practice in medicines management and supports excellence in clinical pharmacy practice in order for practitioners to provide outstanding patient care.

During this time there were a number of key areas within which I worked that have continued on into my current post. Specific experiences gained from this were:- Introduction and development of a new community based anticoagulation service- Provision of regular anticoagulation clinics and telephone dosing for domiciliary patients- To manage administrative staff and team phlebotomists for anticoagulation service- Training of new staff and ensuring on-going competence of all anticoagulation practitioners- Daily provision of prescribing support services to 6 Practices- To monitor practices performances against locally agreed guidelines and targets- To support the PCTs work plan around the National Service Frameworks and NICE guidance- To interpret PACT data for prescribers, to support cost effective prescribing- To influence practice prescribing budgets- To horizon scan for assessment of impact of new drugs or guidelines on practices- To monitor the prescribing of new drugs- To develop, implement and monitor risk management strategies- PCT prescribing lead on the implementation and development of cost savings and ScriptSwitch- A knowledge of “Care in the chemist” schemes within the PCT- Lead role in developing protocols/policies/guidelines for use across all prescribing support teams- To engage in cross-organisational work with other prescribing advisers and pharmacists within secondary care,in areas of mutual interest- Extensive audit of repeat prescribing systems and development of guidelines for PCT staff and independent General practitioners- Network of links professionally, clinically and managerially within the PCT- To promote good practice via the clinical governance, risk management agenda.- Professional development, aware of PCTs developments, strategy and structure and wider NHS policies.
