The Transitions South Office aims to improve the life chances and life choices of homeless young people and unaccompanied asylum seekers targeted to their complex needs to enable them to live independently whilst maintaining their accommodation.I conducted welfare checks, health and safety checks, as well as residents' at-risk checks to service users aged 16 to 18 living in core accommodation. Moreover, I assisted with record keeping, updated online platforms, and responded to crises or incidents that arised when on duty. As part of the role, I also provided assistance with individual support plans and had 1-2-1 sessions with service users to address their needs.
