During my rotation here I rotated through four different ambulatory care clinics. The first was the anticoagulation clinic where I worked under a collaborative care agreement with providers. In this clinic I counseled patients on warfarin, this consultation included diet recommendations and drugs to avoid due to interactions. I also oversaw dosing; I would receive the patients INR that day from the lab speak with the patient to see if there were any changes since the last visit and then adjust their dose accordingly and transition patients to Eliquis or Xarelto when possible. The second clinic was the diabetes management clinic, where I would counsel patients on lifestyle changes, diet recommendations, medications management (provide MDs with recommendations for patient), and glucose machine demonstrations and administrations. The third clinic consisted of Hypertension management, at the beginning of each patient visit I would take the patients’ blood pressure, then speak with the patient to see if there were missed doses of medication or any other changes like added stress, after speaking with the patient I would then provide lifestyle changes and diet recommendations as well, and last would reach out to their MD to provide recommendation on medication that need to be added or dose changes and labs that may need to done. The last clinic I rotated through was the clinic that specifically only saw the unhoused population of Louisville. Here I counseled patients on their medications, rounded with providers to give recommendation for psych medications and opioid use disorder medications, and observe the administration and counseled on Sublocade
