Onco360 is a pharmacy that specializes in oral oncology medication throughout the United States. I am currently their pharmacist intern where my role is to give patients an in-depth counsel on their specialty drugs, along with report adverse drug events to the manufactures and FDA, run drug interaction reports, reach out to providers regarding prescription clarifications, and provide support to patients and their families throughout the duration the patient is receiving therapy.

APPE ADVANCED HOSPITALDuring my rotation at Owensboro Health Twin Lakes I have attended rounds in the CCU with the providers, during these rounds I make medication recommendations for patients with chronic heart failure, diabetes, hypertension, and post-surgery pain control, these recommendation may including adding medications to a patient's regimen, changing doses, or stopping medications, and counseling patients on new medications or provide reasons on why medications may have been stopped.

APPE ACUTE CAREDuring my rotation at Baptist Health Inpatient Oncology rotation I collaborated and rounded with oncologists to provide supportive care to patients receiving active treatment for cancer; which included prevention and treatment of neutropenic fever, pain management control, disease state specific inpatient and outpatient treatment, and counseled patient on treatment regimens.

APPE AMBULATORY CAREThe Medication Management Clinic at Baptist Health is a clinic in a collaborative care agreement with a group cardiologist at Baptist Health. In this clinic I would check patients INR with a Coag-Sense monitor, adjust doses of warfarin according to INRs, counsel patients on what their diet should consist of while being on warfarin, and create a patient specific lovenox bridging schedule for patients undergoing surgery.

APPE AMBULATORY CAREThe Family Health Centers are 340B entities that provide care to underserved areas all throughout Louisville, KY. During my rotation here I rotated through four different ambulatory care clinics. The first was the anticoagulation clinic where I worked under a collaborative care agreement with providers. In this clinic I counseled patients on warfarin, this consultation included diet recommendations and drugs to avoid due to interactions. I also oversaw dosing; I would receive the patients INR that day from the lab speak with the patient to see if there were any changes since the last visit and then adjust their dose accordingly and transition patients to Eliquis or Xarelto when possible. The second clinic was the diabetes management clinic, where I would counsel patients on lifestyle changes, diet recommendations, medications management (provide MDs with recommendations for patient), and glucose machine demonstrations and administrations. The third clinic consisted of Hypertension management, at the beginning of each patient visit I would take the patients’ blood pressure, then speak with the patient to see if there were missed doses of medication or any other changes like added stress, after speaking with the patient I would then provide lifestyle changes and diet recommendations as well, and last would reach out to their MD to provide recommendation on medication that need to be added or dose changes and labs that may need to done. The last clinic I rotated through was the clinic that specifically only saw the unhoused population of Louisville. Here I counseled patients on their medications, rounded with providers to give recommendation for psych medications and opioid use disorder medications, and observe the administration and counseled on Sublocade

While being employed as a pharmacist intern at Kroger Pharmacy I counseled patients on various medications, gave and counseled on vaccinations, inputted, and dispensed prescriptions, ordered and stocked inventory, called providers for clarifications on prescriptions and took verbal prescriptions, and received and provided transfers to and from other pharmacies.

During my time at Professional Pharmacy, I worked part-time as their compounding technician. This title consisted of compounding hormone replacements, antibiotics, vet medications, and pediatric medications; as well as dispense compounded medications and retail medications, check customers out, and help order and stock inventory.

At McMinnville Drug Center I was a part-time technician, where I inputted prescriptions, dispensed prescriptions, stocked inventory, and checked customers out along with helped size compression stockings and medical equipment (crutches, walkers, and canes).

At F.Z. Webb & Sons Pharmacy I was their technician where I dispensed and inputted prescriptions, ordered and stocked inventory, checked customers out, and bubble packed monthly medications for patients and The Webb House Retirement Center.
