• Collaborated with older adults and their caregivers to provide OT services within private homes, assisted living facilities, memory care units, and memory care facilities. • Completed thorough evaluations to establish client-centered goals with tools including 30-Second Chair Rise Test, Patient Specific Functional Scale, Four Stage Balance Test, Five Times Sit to Stand Test, UE ROM, MMT, and Grip Strength Screening. • Worked with individuals with various conditions including: Parkinson’s disease, post-CVA, post-MI, AKA, sciatica, foot drop, dementia, generalized weakness, cataracts, Type II diabetes, depression, and anxiety.• Communicated with interdisciplinary team to enhance client care including nursing, physical therapy, caregivers, family members, podiatry, and primary care providers.• Enhanced quality-of-life and decreased fall risk for clients through interventions targeting areas such as ADLs, IADLs, functional mobility, dynamic standing balance, fine motor skills, cognition, visual tracking, generalized strength and ROM, sleep hygiene, social participation, and functional transfers.• Provided client and caregiver education for areas such as HEP, health management, adaptive equipment, home modifications, DME, transfer training, stress reduction techniques, and medication management.• Used Raintree EMR to document daily notes, progress notes with modifications PRN to intervention plans, and discharge notes with resources provided to maintain functional gains from rehabilitation.• Created a handout that was distributed to clients and caregivers with local, cost-effective resources for areas including assistive technology, social participation, health management, community mobility, and IADLs.
