Postdoctoral Fellow In Pharmacogenetics And Therapeutic Drug Monitoring
Department of Genetics & Genomics Sciences• Initiated, developed and conducted project on personalization of Tamoxifen therapy based on genetic testing and therapeutic monitoring of Tamoxifen and its active metabolites• As a first developed and reported LC-MS/MS based method to therapeutic monitoring of Tamoxifen and its metabolites, including number of their hydroxylated isomers having strong clinical implications (Anal Chem 2010; 82:10186-93). Additionally, performed analytical and clinical validation of this methodology and was responsible for preparation of all the documents for the submission of an application to the New York State Department of Health for approval of this assay for diagnostic use. • Designed and conducted clinical trial on Tamoxifen dose adjustment based on patient’s CYP2D6 genotype and levels of Tamoxifen and Tamoxifen’s active metabolites. As a first showed that therapeutic level of metabolically active form- Endoxifen can be achieved even in patients with a poor metabolizing phenotype by standard dose increase of only 10 mg/day (Clin Pharm & Therapeutics. 2011; 90: 605-11). • Performed CYP2C9 and VKORC1 genotyping of patients with various Warfarin doses to establish/confirm impact of polymorphisms in these genes on the achieving therapeutic range.• Determined new genetics variants in CYP2C9 and VKORC1 across different population and their impact on performance of existing Warfarin treatment algorithms (Pharmacogenomics. 2009; 10: 1243-55)• Compared published gene based Warfarin dosing algorithms in a racially diverse, multiethnic population to improve efficacy and reduce adverse effects that occur during traditional anticoagulant therapy guided only by clinical variables (J Thromb Haemost. 2010; 8:1018-26).

• Clinical and laboratory training biochemical genetics and diagnosis of inherited metabolic disorders using i.e.: quantitative amino acids analysis by Biochrom, LC-MS/MS; organic acids profiling in urine by GC- MS; carnitine and acylcarnitinine analysis by LC-MS/MS; Phorphyria by LC-MS/MS; fluorometric enzyme assays for Fabry, Gaucher and Tay- Sachs diseases etc.• Daily responsibility included biochemical testing and resolution of clinical cases: 1) Preparation of the sample for analysis, 2) Performing analytical testing of the sample: programming and running of the instruments, chromatographic baseline review, mass spec and chromatography peak identification, performance of manual assays. 3). Interpretation of test results: interpretation of mass spectrometry profiles and patterns, evaluation of the clinical significance of findings, gathering additional clinical and/or laboratory data, generating a differential diagnosis, recommending further testing, and generating a draft and final version of the written report, 4) Communication of results to health care providers or ordering physicians.• SOP preparation and verification, QC monitoring, proficiency testing, laboratory equipment monitoring, laboratory management, trouble-shooting and problem solving with assays and instrumentation.• Responsible for development of new mass spectrometry methods for therapeutic drug monitoring (Tamoxifen, Warfarin).• Performed analytical and clinical validation of LC-MS/MS methodologies for NY State Department of Health approval, and accomplished integration of the NY State approved assays into the clinical work flow.•Experienced in the preparation of the laboratory for NY State and CAP inspections.
