Collaborates with physicians to assess, diagnose, plan and implement medical care for patients. Performs duties within the full scope of licensure; responsible for assessing, coordinating the delivery and evaluation of care, including direct patient care, patient/family education and care transitions. Provides medical consultation, including performing supervision of physician delegated treatment procedures, assists with various procedures, provides patient education, treatment and follow-up, may complete daily rounds and provides a continuum of care throughout the continuum of care using an organized multidisciplinary team approach as needed.
