After a few years of building workforce solutions in developing countries, I’ve devoted the past 15 years to advancing proactive, value-based care in the US.In 2012 I started City Health Works in Harlem, NY, with a mission to help people with chronic conditions gain the knowledge, skills, and confidence to take control of their health by building a new workforce of locally hired, highly trained health coaches.City Health Works delivered industry-changing results, including a 7:1 ROI, in the deployment of technology-enabled, clinician-supervised health coaches delivering enhanced primary care support to vulnerable patients. City Health Works’ local hiring model gained national attention, with features in Politico, PBS Newshour, NPR, the Economist, Brookings, and more.In 2020, my team and I acquired a large, 20-year-old independent multi-specialty practice in Washington Heights, NY, and founded CHW Cares. After three intensive years of focus on transforming the practice from fee-for-service into value-based delivery, we sold the medical assets of CHW Cares to Oak Street Health.Diverge Health, a company I co-founded in 2023, acquired the IP of CHW. I work with talented colleagues to scale that work ― defining and refining population health solutions in the rapidly growing markets we serve. The work we're doing inspires me for the possibilities ahead because I know together, we can shape the future of care in transformative ways.When I am not building better healthcare, I am boxing, biking, or singing kirtan with my three boys.
