The mission of HUD-VASH is to support the goal of ending Veteran homelessness by assisting vulnerable homeless and at-risk Veterans in obtaining and sustaining permanent housing. HUD-VASH utilizes the principles of Housing First to support each Veteran in achieving their self-identified goals and improving their quality of life.

Through the HUD-VASH program, HUD and VA increase access to affordable housing for homeless veterans and provide the support necessary to obtain and maintain permanent housing in the community.

• Supervise unit of seven to 10 direct reports who collectively perform all functions of theCOVID-19 Community Team response• Evaluate performance and provide one-on-one mentoring sessions with team members• Serve as primary point of contact for people newly diagnosed with COVID-19 and initiatecontact tracing and surveillance

• Manage a caseload of students in Post-Secondary program• Provide individualized post-secondary advising and coaching students to navigate challenges and focus on problem-solving, particularly around situations commonly faced by first-generation, low-income college students• Advise students on academics, financial aid and money management, engagement on campus and in the community, and career exploration, as well as providing access to other resources as necessary• Utilize Salesforce database to track key data and monitor progress • Collaborate to identify and address risk factors for college matriculation and attrition

• Complete biopsychosocial assessments and treatment plans• Manage a caseload of long term and brief psychotherapy clients• Facilitate weekly out-patient psychotherapy utilizing trauma-informed brief therapy including Cognitive Processing Therapy (CPT) and Acceptance and Commitment Therapy (ACT)• Co-facilitate health and wellness groups on acute inpatient psychiatric unit for individuals in recovery who struggle with psychosocial stressors such as homelessness, suicidality, as well as single, dual, or multiple diagnoses such as post-traumatic stress disorder, substance use disorders, or psychosis• Advise on case management, crisis intervention, and discharge planning• Collaborate within interdisciplinary teams

• Advocate for members of the Commonwealth of Massachusetts who are applying for disability retirement• Provide informational support to members while managing and processing all aspects of accidental and ordinary disability retirement applications and benefit payments• Perform factual investigation on disability cases and obtain required documentation • Collaborate with members of the disability team as well as legal team to mediate any discrepancies found during the application process

• Utilize clinical social work techniques with high-risk patients and their families regarding the impact of emotional and social problems• Conduct psychosocial assessments and assist in discharge and treatment planning within a collaborative multi-disciplined team• Coordinate case management services at time of discharge• Participate in Grand Rounds with Psychiatry and Child Protective Services

• Provided intensive case management services to homeless and at-risk client• Provided information and referral services, and brief counseling around immediate client concerns and/or barriers to housing, including financial instability, legal issues, mental health issues, substance abuse, un- or under-employment, health care, and housekeeping• Obtained resources and services as requested and/or needed to address clients’ needs, including housing applications, negotiation and mediation with property owners and utility companies, fuel assistance, and shelter information• Initiated and maintained communication with other community agencies, shelter providers and state agencies serving clients to ensure that needed resources are available to ensure permanent housing placements and long-term housing stability

• Provided advocacy and care management services to Primary Care patients• Assessed patient needs and barriers to care to assist in the development of a plan of action • Connected patients to social service benefits while monitoring progress and advocating as needed

• Handled inbound and outbound communications for BMC’s Primary Care Practice and Geriatrics. • Handled scheduling and rescheduling appointments, following up with patients pre-visit and post-care, medication reconciliation, refills and insurance verification and authorization management. • Communicated with patients and staff.
