To liaise with GP Surgeries, Hospitals, Social & Mental Health Services, District Nurses, Relatives, as well as numerous other MDT members to promote and advocate for the best practicable care provision for residents.To bring previously learnt and practiced skills into the home, as well as maintaining knowledge of current relevant research and advocate its application in practice.Monitor staff compliance home policies and the homes compliance with relevant CQC regulation. Completion of relevant CQC reporting regulation as required.Regular professional supervision and continuing personnel development reviews of qualified staff.Effective reporting and communication with Line Managers and the multi-disciplinary team regarding patient care, incidents, health and safety issues and service deficits.Continue professional development in a participated and constructive manner. Setting objectives through Clinical Supervision with the aim of developing skills and abilities.Develop effective liaison and maintain good relationships with other professional disciplines and with external statutory and voluntary agencies.To participate in any formal monitoring, review or quality systems developed by the home. This could include clinical supervision, individual performance reviews and standard setting.To work as a member of a multi-disciplinary team in the assessment of patients, care plans and discharge arrangements.Be conversant with the NMC Professional Code of conduct, CQC Guidance and Care Standards as well as other relevant regulations applicable to private nursing and private health care. Uphold professional standards and conduct at all time.
