Work as part of a team that utilizes a Harm Reduction/Housing First model to assist people who have experienced long-term homelessness (with a primary focus on women and people who identify as female) in finding and maintaining housing. Respond to each participant's unique needs and goals by providing on-going support, advocacy and assistance in accessing community services to ensure stability in housing and integration in the community.Knowledgable about and able to respond effectively to the complexities of participants' lives and the co-occurring challenges and conditions associated with long-term homelessness. Handle a caseload of 15-20 participants that have experienced long-term homelessness located in scattered site housing throughout the Hennepin County area. The participant populations include those with chemical abuse issues, criminal histories, trauma, grief and/or mental health. Provide mentorship to new employees as assigned.Provide leadership on team, providing positive support to team members as a senior member.Conduct persistent outreach to potential participants within relevant shelters, building trusting respectful relationships with hard to engage participants. Network with other agencies and individuals to access housing.Cultivate relationships with landlords willing to provide housing for participants.Assist clients with completion of necessary forms to access housing.Assist participants to secure and maintain housing, apply for benefits and assist with accessing and moving furnishings.Offer regular and quality contact with participants to maintain trust, to assist in crisis prevention and assist in meeting individual needs. Maintain individual files, including case notes, signed data privacy, intake and discharge summaries, and releases of information.Ability to work with individuals from very diverse backgrounds.Ability to promote a professional work environment that is affirming, respectful and understanding of diverse people.

Relational concerns include conflicts and cut-offs with family of origin, as well as conflicts within the residential community.
