Care Coordination is the cornerstone of Centennial Care with members receiving the resources and services they need to live healthy, independent lives. My team conducts regular audits of the Managed Care Organizations for member engagement, timeliness of care coordination requirements and member issues. We conduct trainings on documentation, care coordination requirements and best practices with our diverse population across the state. Listening to our members needs and connecting members to the services, resources and quality care they are eligible for is our daily mission.

Work with the AAA's (Area Agency's on Aging) in overseeing the SSRD, a resource directory available for citizens across New Mexico. Support the work of the ADRC in connecting individuals with needed and available services. The New Mexico ADRC is the first state to begin a chat feature for ease of access to services and the Web/I&R Data team run this exciting program. Our constant goal is to provide the best service to all of the state's citizens and give them the options to make informed choices for their long term needs.
