It employs approximately 5,500 people with the primary purpose of delivering expert, evidence-based, high-quality care to the patients it serves.I have several roles within this position: ✔ Throughput Management: Ensuring appropriate and steady patient flow through our acute inpatient hospital.✔ Discharge Planning: Working with the patient and his or her significant others to identify and prepare for patients’ anticipated health care needs after they leave the hospital.✔ Transition Management: Ensuring ongoing support of patients and their families over time in their communities as they navigate care and relationships among more than one healthcare provider. ✔ Root Cause Analysis (RCA) of readmissions: Using the WSHA-endorsed Aspire tool to identify the root cause of readmits, focusing on putting interventions in place to tackle those reasons, preventing future avoidable readmitsIn this role, I have been able to:✔ Lead a team of colleagues in creating a process around incorporating the WSHA-endorsed ASPIRE tool into our everyday work processes to assist with Root Cause Analysis of Readmissions. ✔ Develop a Provider checklist to speed up identification of the Legal Next of Kin so that the patients LNOK is identified sooner.✔ Submit to department leadership multiple process improvement ideas, including how to reduce moral distress in case managers, how to lessen the burden of extensive chart review for case managers and how to ensure transparency around patients’ insurance benefits for all providers.✔ Became the first person on my team to become certified in case management through the National Board for Case Management.✔ Orient and mentor new staff to help them become successful.

My purpose as the red thread was to ensure that the patient and the rest of the team were all on the same page regarding the goals of care and the plan to reach those goals reducing fragmentation of care , inefficiency of process, and patient/family dissatisfaction.✔ Facilitator of team conferences and family conferences to clarify next steps needed to help the patient and their families reach their health care goals.✔ Care Coordinator, working with patients and their families to help manage their care while the patient remained an inpatient and set up support services for when patient was dischargedIn this role, I was able to:✔ Lead a team of colleagues in creating a template called the Transitional Plan of Care (TPOC) that addressed all the transition-related needs that a patient may have on being discharged from the hospital.✔ Lead a team of colleagues in creating a process around the use of care protocols to guide the team in caring for patients and their families whose behaviors interfered with the clinical care been given to the patient (examples of those behaviors were physical and verbal aggression, illicit drug use, staff splitting, drug seeking, and malingering).

Then I became part of the Short Stay Surgical Team that delivered high-quality care to patients admitted to out Short Stay Surgical Unit.

Al Hada Armed Forces Hospital is a tertiary healthcare facility in Taif, Saudi Arabia, that delivers high quality, integrated, evidence-based, safe clinical healthcare to military and civilian employees. While at Al Hada, I delivered culturally appropriate bedside care for female-only surgical patients. I also had the opportunity to work as the Interim Nurse Manager while the Nurse Manager was on vacation.
