•Schedules and completes all assigned 30-50 assessments with the members and, if appropriate, care team in the timeframe required• Conducts assigned assessments in-person at the individual’s residence or current facility• Responsible submitting certifications to the county to initiate eligibility• Monitors the status of financial eligibility for wavier approval• Ensures all initial required paperwork (PMIP, ROI, signature pages, etc.) is completed and updated in the member’s profile in the state operated system• Provides updates and information to the member and their care team about service delivery options and resources as needed• Keeps the member and their care team updated about the status of their eligibility, including when they can start services or if eligibility requirements are not met within assigned timeframes
