I conduct assessments, identify gaps and create solutions to close the gaps in all areas. In heading the Post Acute Care Performance Network I establish systems for reliable coordination of care. With all these specialities I oversee a Budget of greater than $24M with 8 direct reports and 300 employees. * Organized groups of internal/external stakeholders improving transitions of care across the care settings. All cause readmission rate 8.8%, LOS .5 day improvement for patients requiring facility placement at discharge, ED return to community rate > 55% for patients hospitalized within 30 days.* Collaborated with VP Population Health, formed of a multidisciplinary team, and created a Care Coordinator role to design and manage the CJR 90 Day Bundle; reduced post acute continuum use by 60%, post facility LOS by 50% with overall shared savings of $500K year 1.* Expanded the Ambulatory Care Coordinator role. Realized statistical improvements in quality metrics in diabetes and realized a $2200 cost reduction in patients participating in the program.* Completed a Market Analysis for Home Health, Home and Inpatient Hospice. Hired a Business Development Specialist. Partnered and implemented with the leadership team 20% improvement in productivity, 15% increase in revenue per episode, and 4.5 CMS rating in patient experience and quality.* Implemented, along with IT, CMS TCM program. The goal is to create a continuum of services and systems to achieve the highest clinical outcomes of care at the lowest cost. An innovative matrix model of community services is under development.
