Behavioral Health Practitioner
CurrentConducts clinical and/or psychosocial assessments with individuals with severe mental health conditions to determine their psychiatric and psychological needs.Develops diagnoses and service plans.Provides psychotherapy for individuals, groups and families.Functions as a part of a multidisciplinary team in the field and community. Makes referrals to other community resources for unhoused clients, and intensive or continued clinical services.Contacts public social welfare and other mental health agencies to obtain and provide information.Keeps accurate records of interventions and service plans.Provides education and consultation services to community agencies.Provides outreach services to underserved populations.
