Also, participate in program model development evaluation, and collect and compile data as required by programming model leads.--Participate in regular multidisciplinary case conferences with medical providers as needed. Complete population health projects as assigned and panel-manage patients in collaboration with other behavioral health staff.

--Serves as a mental health resource professional to International Academy at Hull as part of the School Improvement Grant.--Coordinates school-based social/emotional support activities as outlined in the School Improvement Grant.--Develops positive relationships among students, families and school staff at International Academy at Hull to enhance social and emotional health.--Provide education and support to students, teachers, and families in the area of mental health.--Provides support to students through the use of individual and small group sessions and community resources.--Assists students with social and emotional growth as it relates to academic success.--Works to enhance student achievement, promote student attendance and increase student responsibility through addressing mental health and wellness in the school setting.--Develop mental health interventions, maintain records, and make data-driven decisions on program and service modification in accordance with the School Improvement Grant.

--Provides services to children/adolescents with severe emotional disturbances and their families including individual and family therapy and case management. --Prepares comprehensive biopsychosocial assessment, treatment plans and periodic reviews.--Documents treatment and provides clinical guidance as directed to other clinical, direct care staff, and families as appropriate.--Completes brief assessment, crisis intervention and pre-screens as needed.--Provides transportation to consumers as deemed appropriate.
