Case Manager
Current- Managing clients' services: making sure clients are getting the services they need, coordinating services, communicating with service providers, etc.
- Documentation: completing, updating, submitting, and compiling necessary documentation for clients.
- Regularly meeting and communicating with clients (virtually or in-person)
- Act as a liaison between the facility and resources external to the organization.
- Establish and maintain effective cooperative working relationships with social service staff, nursing staff, medical staff, and other departments.
