I am a Wellness specialist, disease manager and case manager. Provide educational resource support to members who have chronic medical health conditions that require assistance in maintaining compliance with their care and doctors appointments. My primary goal is to influence behavioral changes which ultimately result in improved member well being as well as clinical improvement. Development of an individualized care plan in collaboration with member's short, and long-term goals that address issues such as quality of life, satisfaction with care, functional abilities, effectiveness of treatment and barriers to the Care plan. Follows established guidelines specific to conditions management program with respect to documentation and maintenance of patient medical record. Administers initial and monthly follow up calls and documents accordingly.· Function independently, work from home remote position and perform Health Risk Assessments (HRAs) at an efficient level of productivity.· Thoroughly assess and document member health data via electronic platform to ensure the highest quality HRA is submitted · Facilitate relationships with members by completing confirmation calls, honoring reasonable scheduling requests and providing exceptional customer service· Complete documentation and duties within specified guidelines and timeframes. · Engage in health and wellness coaching including evaluating, interpreting and administering personal health assessments, including collection and interpretation of biometrics.· Educate members on the importance of preventive health measures; assist in identifying and resolving barriers to care and care gaps · Facilitate and/or support outreach, community projects or special events, educating on wellness and lifestyle topics · Assist members with understanding benefits, authorization requirements and cost and quality measures. · Efficiently navigate numerous applications and systems to obtain accurate information to assist members
